Program Overview

Objective:

  • To support the establishment of mental health multidisciplinary services within primary care settings.
  • Facilitate employment of health practitioners to optimize support for consumers with severe and/or complex mental health needs.
  • Enable Primary Health Networks (PHNs) to tailor services to local needs and market conditions.

Target Beneficiaries:

  • Consumers with severe and/or complex mental health needs.
  • Primary Health Networks (PHNs) across ACT, NSW, VIC, SA, WA, QLD, NT, TAS invited by the Department of Health.

Key Features:

  • PHNs have flexibility to choose the most appropriate funding and care models.
  • Services may include care coordination, navigation, case management, brief interventions, psychosocial support, and social prescribing.
  • Funding available from 2025-26 to 30 June 2028.
  • Annual grant funding per PHN ranges from approximately AUD 1.3 million to AUD 3.1 million.
  • Total funding pool is AUD 71,256,000.
  • Selection process is targeted or restricted competitive.

Application Process:

  • Only invited PHNs are eligible to apply.
  • Applications must be submitted via the GrantConnect portal by the closing date (7 July 2025, 2:00 pm ACT time).
  • Applicants must submit a completed application form with necessary attachments.
  • Confirmation of submission is sent via automated email; technical support available via GrantConnect Helpdesk.
  • Specific instructions on file naming, file types, and submission limits apply.
